| Outline of Annual Research Achievements |
過敏性腸症候群(irritable bowel syndrome; IBS)は、ストレスによって腹痛と便通異常が増悪する代表的な内科疾患である。本研究の目的は、ストレス関連疾患としてのIBSの病態生理を脳腸ペプチドcorticotropin-releasing hormone (CRH)の動態から明らかにすることである。健常者を対照とし、IBS患者における内臓刺激に対するCRH動態の異常を分析しつつある。また、大腸刺激下でのPETによる局所脳血流による中枢神経応答がCRHの存在下でどのように変化するかを検証した。また、健常者の大腸電気刺激時に内臓知覚大脳誘発電位と血中ACTH、cortisolを分析し、同時に左右の背外側前頭前野に経頭蓋磁気刺激を加えてその影響を見ている。CRH受容体CRH-R2の刺激はストレス応答を減弱させることが知られている。IBS患者と健常者のCRH-R2遺伝子を含むゲノム分析を行った。動物はラットに大腸伸展刺激を加える前に音刺激を加え、内臓刺激による恐怖条件付けモデルを作成し、扁桃体のCRH陽性線維の変化を検討した。結果は、大腸刺激に対するACTH、cortisol分泌は増加、不変、減少の３群に分離した。ACTH増加群は、CRH負荷後の高い内分泌反応、強い腹痛と腹部不快感、高いストレス評価を示した。一方、ACTH減少群は、高い不安感受性を特性として持ち、予期不安から大腸刺激前に既に内分泌反応とストレス評価を強め、大腸刺激後にむしろその反応性を弱めた。IBS患者では扁桃体局所脳血流量がCRH投与で増加した。IBS患者ではこれに内臓刺激が追加されても扁桃体活動が飽和しており、これが血漿noradrenalineの動態と並行した。内臓刺激による恐怖条件付けモデルの作成に成功し、その病態は扁桃体中心核のCRH陽性線維の増加を伴っていた。以上の結果を順次論文化して公刊して行く予定であり、今後の研究の展開が多いに期待される。

申請書に於いて、脳画像を用いた脳腸相関へのCRHの関与の分析、遺伝子多型分析、磁気刺激、動物実験について計画を立案している。研究実績の概要に記載した通り、申請書に記載した速度で分析が進んでいる。
特に予期せぬ不都合は生じていない。

申請書に於いて、脳画像を用いた脳腸相関へのCRHの関与の分析、遺伝子多型分析、磁気刺激、動物実験について計画を立案している。研究実績の概要に記載した通り、申請書に記載した速度で分析が進んでいる。
当初の計画通り進展させることにより研究を進める方策で良いと判断する。
