Outline of Annual Research Achievements |
簡便な肝線維化脂肪化診断法についての臨床研究を行った。各機種間の相関と肝線維化のゴールドスタンダードになっている機種への変換式が可能であるかの検討を肝生検と同時に施行した126名を対象とし、超音波診断装置5機種と肝硬度測定専用機(ACUSON S2000/3000、 Aixplorer、 iu22/EPIQ7、 AplioTM500、 LOGIQ E9 とFibroScanで肝硬度を測定した。 (1)平均年齢57.7歳、HBV/HCV/B+C/NBNCはそれぞれ23/63/1/40であり、 線維化stageはF1/F2/F3/F4: 54/24/32/17であった。各機種ともにF stageの上昇とともにvelocity(Vs値)も上昇し、肝硬変診断能はAUC0.95-0.98と非常に高い診断能であった。相関係数が高く回帰式の作成が可能と考え、それぞれ、FibroScanへの変換式を検討した。(2) 肝硬度測定値との関係を検討するために、VTQ(Vs値)と有意な相関関係を示す因子を検討した。F因子、Plt、アルブミン、PT-INRであり(p<0.001)、特にF因子との相関が最も強い傾向であり、肝硬度測定値は肝線維化診断に用いられることが確認された。FibroScanとそれぞれのSWE機器の間に回帰式を使ってFibroScanへの変換式を求めた。①FibroScanとSWE-Aixplorer :TE (m/s) = 1.23 × SWE-Aixplorer (m/s) - 0.40②FibroScan とSWE-EPIQ : TE (m/s) =1.10 × SWE-EPIQ (m/s) + 0.06③ FibroScan と SWE-LOGIQ : TE (m/s) = 1.82 × SWE-LOGIQ (m/s) - 1.18④ FibroScan とSWE-ACUSON: TE (m/s) = 1.45 × SWE-ACUSON (m/s) - 0.30.⑤ FibroScan と SWE-Aplio :TE (m/s) = 1.29 × SWE-Aplio (m/s) - 0.45.今後本法を用いて非侵襲的検査法に替わる肝線維化診断法として広く普及することが望まれる。
