Outline of Annual Research Achievements

脳体積研究については、45例の大うつ病性障害患者、年齢、性、教育歴が一致した61例の健常者が参加した。参加者はMRI撮像し、灰白質体積をvoxel-based morphometryおよび白質走行をdiffusion tensor imagingにより撮像しtract-specific analysisにて解析した。 その結果、うつ病患者は、健常者と比べて、眼窩前頭前野、前部帯状回、島領域、扁桃体、側頭部の灰白質体積が小さかった。また、大うつ病性障害患者は健常者と比較し、鈎状束の異方性が有意に高かった。大うつ病性障害患者においては、うつ病の重症度が高い患者ほど、眼窩前頭前野および島領域灰白質体積が小さかった。また、眼窩前頭前野が小さいほど鈎状束異方性が高かった。以上のことから、大うつ病性障害の眼窩内側前頭辺縁ネットワークにおける灰白質体積と白質連結性と関連があり、このネットワーク分断がうつ病の病態に関与していることが示唆された。
また、遺伝的要因と脳体積との関係を調べるため、気分障害の家族研究を行った。双極性障害群（10例の双極性障害患者、10例の双極性障害患者非罹患家族）、大うつ病性障害群（17例のうつ病患者、17例の双極性障害患者非罹患家族）、27例の健常者の比較を行った。その結果、双極性障害群は、前部帯状回および島領域灰白質体積が小さかったが、大うつ病性障害群では島領域灰白質体積が小さい傾向が見られた。この結果から前部帯状回体積異常は双極性障害のエンドフェノタイプの候補となり、島領域は気分障害に共通の病態を示すが、双極性障害でより重症であることを示した。
さらに、うつ病患者の白血球から抽出した遺伝子発現のいくつかが健常者ではみられないことを発見した。

Reason

今年度は、MRI脳体積の解析をすすめることができた。うつ病患者で灰白質体積と白質走行異常が同時に障害されていることを論文化することが出来た。さらに、MRIの気分障害家族研究も進めることが出来た。また、うつ病患者の末梢血からの遺伝子発現でこれまで報告されていない遺伝子異常をみつけることが出来た。

Strategy for Future Research Activity

他の施設から提供された1500例のMRIデータを用いて、うつ病と双極性障害の灰白質体積減少の異同について現在解析中で、論文化していく。
また、被験者の白血球から得らえた遺伝子発現をうつ病患者と健常者と比較し、うつ病モデルマウスで得られるマイクロアレイデータからの遺伝子発現との共通遺伝子があるかどうか比較し、論文化していく。

Causes of Carryover

MRIデータ集積および解析を中心に行い、血液データ解析の試薬量が少なかったため、当初の計画より少ない予算執行となり、次年度使用額が生じた。

平成29年度は、血液データ解析およびその成果の学会発表、論文投稿および掲載料に充てる予定である。
