Outline of Annual Research Achievements

本年度は,以下の項目の研究を引き続き遂行した.
(1)各撮影部位(頭部,頸部,胸部,腹部)における各臓器被ばく線量の定量的な評価
患者CT画像を用いて,対象とする臓器の輪郭を描出した.次に,臓器の輪郭情報を含むCT画像をDICOM出力でモンテカルロ計算コードに取り込んで線量分布計算を行った.各部位の臓器線量は,線量体積ヒストグラム(DVH)を用いて定量的に評価した.本研究では,各社CT装置(LightSpeed VCT(GE社),AquilionONE(TOSHIBA社),SOMATOM Sensation(SIEMENS社),Brillance iCT(PHILIPS社)の被ばく線量を同一撮影条件においてDVHからの平均吸収線量で算出した.各社CTの120 kVにおけるAl半価層(Philipsで9.1 mm,Siemensで8.6 mm,GEで 7.5 mm,東芝7.2 mm)にしたがって,臓器平均線量は減少した.臓器線量は線質に大きく依存することが確認できた.

(2) 各社CTの自動露出機構(AEC)の装置間比較
以前に作成したAEC測定用ファントムを用いて,ヘリカルスキャンから上記の各社CT装置におけるX-Y軸面(Axal面)とZ軸のAECの特性(管電流,画像ノイズ,線量分布)を調べた.Axal面-AECではモンテカルロ計算によるモデリングも行った.Axal面-AECは各CT装置で同様な管電流変調が行われており,測定値とモンテカルロ計算値は凡そ一致した.一方,Z軸-AECではファントムサイズや形状に応じて,各社CT装置によって管電流変調に大きな違いが見られた.現状では,モンテカルロ計算においてZ軸-AECを一定のパラメータで予測することが困難であり,各スキャンの各照射角度での管電流値がDICOMで取り出せないと正確なモンテカルロ計算は困難であると考えられる。
