脳梗塞発症後のマクロファージ活性化とその制御による脳保護作用

Outline of Annual Research Achievements

脳梗塞発症予防効果に対する研究において、申請者らは脳梗塞のリスクファクターに関連した治療薬を用いて脳損傷抑制効果や、メカニズムをこれまでに報告してきている(Stroke 2009, 2011, Hypertension 2012, Brain Research 2015など)。また脳梗塞発症後の治療に関して、血栓溶解のために適応症例に用いられているt-PAに対してラジカルスカベンジャーのエダラボンとの併用がtPAによる出血作用を抑制することやそのメカニズムを明らかにしている(Stroke 2009)。近年、脳梗塞後の炎症や免疫作用に関連した報告が多くなっているが、マクロファージの役割についての詳細な検討は十分なされていない。マクロファージは単球から分化し、生体に侵入した細菌やウイルスなどを貪食・消化する。主にM1マクロファージ、M2マクロファージの2種類のタイプに分けられる(右図)。M1マクロファージはIFN-γやTNF-αなどを産生し、細菌や腫瘍細胞に対する殺細胞作用や炎症性作用を示す。これに対し、M2マクロファージはIL-10やTGF-βなどを産生し、M1マクロファージのような細胞傷害機能は持たず、細胞増殖や血管新生促進因子を発現することで、虚血後の脳損傷部位で神経再生に働く可能性が考えられている。またM2マクロファージは免疫抑制作用を示す。このようにマクロファージは賦活化されるタイプによって作用が異なる可能性があるため、マクロファージの脳虚血後の作用をよりわかりやすくするためにマクロファージ賦活作用のあるGcMAF(Gc protein macrophage activating factor)を用いてその影響や分子機構について解明することが重要と考えた。

Reason

脳虚血後急性期および安定期(早期と後期)のペナンブラ領域におけるマクロファージの発現(M1/M2マクロファージの比率)を免疫染色や定量real-time PCRにより解析し、M1/M2マクロファージの役割が脳梗塞後の時期によって異なることが判明した。

Strategy for Future Research Activity

脳梗塞後期にM2マクロファージを賦活する意義を明らかにすると共に急性期にM1マクロファージを制御する方策について検討する。M1マクロファージの発現が高く、脳梗塞が拡大する急性期に抗炎症作用物質を前投与し、GcMAFと併用した場合の影響を評価する。
