Outline of Final Research Achievements

The purpose of this study is to create a social inclusion-type life management program（SLMP）for elderly people with chronic heart disease. We conducted an interview investigation and questioners survey in the main caregivers with elderly people with chronic heart disease during home care.
It was guessed that it was to have a communication tool that the factor to be made results, social ink slovenly utilized not being that independent, a main caregiver cared for regardless of cognitive function and ADL (single support), Internet environment and was connected with others.
