Outline of Annual Research Achievements

我々は、卵胞刺激ホルモン(FSH)がcGMPシグナルを介して顆粒膜細胞を増殖し、卵胞を発育させる分子基盤を明らかにした。現在のところ、加齢などによりFSH刺激に低反応で十分な卵胞発育が得られない不妊患者に対して、卵胞発育を促進させる有効な治療法はなく、治療に苦慮することが多い。そこで、本研究は、顆粒膜細胞内でcGMPを分解するphosphodiesterase 5 (PDE5)を標的とし、PDE5の機能を一時的に抑制することで、これらの患者の新たな卵胞発育法の開発を行うことを目的とした。
これまで、PDF5抑制剤であるSildenafilに加えてTadalafilが、in vitroおよびex vivo試験において、卵胞発育を促進することを見いだした。その効果はTadalafilの方がSildenafilよりも優れていた。また、46-50週齢の高齢マウスに対し、SildenafilおよびTadalafilを投与したところ、血中cGMPの上昇とエストロゲン値の上昇を認め、排卵数が増加した。このin vivo試験においても、Tadalafilの方がSildenafilよりもより排卵数を増やす効果を認めた。Tadalafilを投与マウスから得られた卵子の正常性を検証するため、体外受精胚移植を行ったところ、非投与群と比較して、成績に差を認めなかった。さらに、Tadalafil投与群から得られた児および胎盤にも異常は認めなかった。また、SildenafilおよびTadalafil投与マウスかた得られた卵子の遺伝子発現を非投与群と比較したが、特に差違を認めなかった。ヒト顆粒膜細胞、卵巣組織の培養系において、SildenafilおよびTadalafilの効果を検証したが、細胞培養では有効性が認められたが、組織培養では有意な卵胞発育は得られず、さらなる解析が必要と考えられた。
