外傷初期凝固線溶系変化の病態生理解明

Outline of Annual Research Achievements

2015年度の研究で外傷性凝固障害の病態は凝固亢進、凝固制御機能不全、線溶抑制であり、播種性血管内凝固症候群（disseminated intravascular coagulation, DIC）の病態生理に一致する事を確認した。この研究成果に基づき本研究の第二の目標である多施設共同前向き研究を計画し実行に移した。日本救急医学会が公募する学会主導研究に応募し同学会の承認を得た。北海道大学医学研究科救急医学分野が研究主幹校として多施設共同前向き試験を計画し、北海道大学病院自主臨床研究委員会に研究計画書「重症外傷の疫学的研究」を提出して同委員会の承認を得た。研究目的は、
1）重症外傷の疫学、
2）重症外傷の病態生理と臓器不全発症機序（凝固障害を含む）、
3）重症外傷の診断・治療方法であり、救急室に搬入された外傷患者および入院中の外傷患者を対象として行う前向き観察研究（検体による探索的研究）
である。主要評価項目は28日死亡率であり、副次的評価項目を外傷性凝固障害（DIC）の病態生理と臓器不全発症機序に設定した。この研究計画書承認に基づき2015年末に日本救急医学会専門医指定施設に対して研究参加の公募を開始した。2016年3月までに83施設の研究参加があり、2016年4月から2年の予定で症例登録を開始し、2017年3月末の時点で約250症例が登録された。本研究は2018年3月31日まで症例登録予定（研究期間2年）であり、現時点で研究実績を公表するに到っていない。

Reason

自施設での研究は臨床および基礎研究ともにおおよその結果を出すことが出来た。この結果に基づき次の目標である多施設共同前向き臨床研究を計画し、実施に向けての活動を開始した。単独施設ではなく多施設共同前向き研究であるため、研究参加各施設の倫理委員会承認に多くの時間を要した。参加全施設の倫理委員会承認を確認して研究を開始する必要があるが、この全参加施設倫理委員会承認に時間を要した事が多施設共同前向き研究開始の遅れの原因と考えられる。

Strategy for Future Research Activity

北海道大学病院が研究主幹校として研究公募に応募した83施設で多施設共同前向き研究を実施中である。本研究は2016年4月に開始し、2018年3月に終了予定である。

Causes of Carryover

多施設共同研究を計画し実施に向けての活動を開始したが、研究参加全施設の倫理委員会承認に時間を要した。このため研究開始が遅れ未使用額が生じた。

Expenditure Plan for Carryover Budget

2016年度は研究計画に基づき、研究検体測定費用として1138100円を計上し使用した。2017年度は既に仕様書を北海道大学医学部外部資金を通じて北海道大学調達課へ仕様書を提出済みである。この仕様書に準じて2017年度交付金額を使用予定である。
