食事調査における申告誤差が食習慣と健康指標との関連に与える影響:日韓英米比較研究

Outline of Annual Research Achievements

前年度に引き続き米国の全国食事データセットを用いて、小児における1日あたりの食事回数・間食回数と食事の質および肥満との関連の検討を行った。24時間食事思い出しデータをもとに、食事および間食回数を計算した。6~11歳の男女(4346人)において、間食回数はその定義にかかわらず、過体重および腹部肥満と正の関連を示した(食事回数は関連なし)。一方、12~19歳の男女(6338人)における食事・間食回数と肥満との関連は定義によってさまざまで、はっきりとした結論は得られなかった。食事の質との関連においては、食事回数とのあいだの正の関連が両年齢階級で観察された一方、間食回数とのあいだの関連は間食の定義によってさまざまであった。
英国の全国食事データセットを用いて食事および間食のエネルギー密度と食事全体の質との関連を検討したところ、成人(1451人)と小児(1617人)の両方で食事のエネルギー密度と食事全体の質とのあいだに有意な負の関連が観察された。また、成人においては食事のエネルギー密度と肥満度および腹囲とのあいだに有意な正の関連がみられた。
国民健康・栄養調査のデータを用いて、1~19歳の小児(3866人)におけるエネルギー摂取量の申告誤差を検討した。エネルギー摂取量を推定エネルギー必要量で割った値の平均値は1.04と、集団代表値としてはおおむね良好なエネルギー摂取量推定値が得られていることが確認された。また、20歳以上の成人(15618人)における食事のエネルギー密度を算出したところ、平均値は男性で5.98 kJ/g、女性で5.72 kJ/gであり、欧米諸国で観察される値よりもかなり低いことが確認された。また、エネルギー密度が低い食事は果物や野菜、微量栄養素が多く、飽和脂肪酸が少ないといった栄養学的に望ましい食事摂取状況と関連していたが、高ナトリウム摂取とも関連していた。
