Effects of measurement errors of dietary intake on the association between dietary habits and health outcomes, using dietary survey data in Japan, Korea, the UK, and the US

Outline of Annual Research Achievements

最終年度は主に日本の国民健康・栄養調査のデータを用いて以下の研究を行った。1)20歳以上の成人(19986人)から得られた食事記録におけるエネルギー摂取量の申告誤差を検討した。エネルギー摂取量を推定エネルギー必要量で割った値の平均値は0.98と、集団代表値としてはおおむね良好なエネルギー摂取量推定値が得られていることが確認された。2)成人(15618人)において、食事性酸性負荷と肥満指標および血圧とのあいだには弱いながらも統計学的に有意な正の関連があることを明らかにした。3)食事のグライセミック・インデックスの分布や食品・栄養素摂取量との関連を明らかにした。4)国民健康・栄養調査で収集された1日間食事記録(26361人分)に登場した94439回の食事を、食品の食べ合わせに着目して、頻出アイテム集合マイニングをもとにした手法を用いてコード化するシステムを確立した。さらに、英国のデータを用いて、エネルギー摂取量の申告誤差が食事由来の温室効果ガス排出量と食事の質との関連に与える影響を明らかにした。
研究期間全体を通じて行ってきたのは、日米英の全国食事調査をもとにして、すべての食事調査における深刻な問題であるエネルギーの申告誤差が食習慣と健康指標との関連にどのような影響を与えうるのかということである。その結果、エネルギー摂取量の申告誤差が与える影響は食事変数によって異なり、エネルギー摂取量と強く関連する変数(例、摂食回数)では比較的大きく、エネルギー摂取量との関連がそれほど強くない変数(例、グライセミック・インデックス)では比較的小さい、ということが明らかになった。いずれにしても、食事調査における申告誤差は栄養疫学における結果に無視できない影響を与えうるので、あらゆる食事調査において申告誤差の程度を評価するための方法論を組み込んでおく必要があるといえる。
