Outline of Annual Research Achievements

ヒト臍帯血よりセルソータで分離した巨核球前駆細胞を用いて,単一細胞レベルで遺伝子発現解析を行った.その結果,未分化な造血幹細胞分画(CD34+, CD38+, CD45RA-, CD90+)に含まれる細胞中に,巨核球関連遺伝子であるcMpl, GPIbα遺伝子の発現を認め,一部の細胞にはvWF遺伝子も発現していた.この結果は,我々のグループがマウスにおいて同定した巨核球前駆細胞(Nishikii, Stem Cell 2016)の遺伝子発現と類似しており,同細胞がヒトにおけるカンターパートである可能性が高い.現在,同細胞分画を用いた分化能の解析を行っている.
成人造血器腫瘍15例に対して,当施設倫理委員会の承認を得た上で臍帯血骨髄内移植を行った(UMIN000006175).移植後の血小板の回復(5万/mcl)は13例(87%)に認められ,その中央値は移植後45日(範囲32-87日)であった.一方で対照群としての臍帯血静脈内移植後の血小板回復は58%,中央値78日であり,血小板回復は骨髄内移植群で有意に優れていた(P=0.007).一方で,好中球生着は,骨髄内移植と静脈内移植で差を認めなかった(P=0.19).また骨髄内移植法を行った症例に対し,移植後早期(14日),血球回復後(28日)に輸注局所(腸骨骨髄)および遠隔部位(胸骨骨髄)より検体を採取し,両検体のドナーキメリズムを解析した.その結果,移植後早期に局注入部位のドナー造血が亢進していた(P=0.006).これらの結果から,ヒト臍帯血中に血小板分化に傾いた造血幹細胞(巨核球前駆細胞)が存在し,骨髄内移植された局所に生着することで移植後の造血(特に血小板産生)に与ることが示唆された.
