くも膜下出血後早期脳損傷の病態解明

Outline of Annual Research Achievements

テネイシンCノックアウトマウスと野生型マウス(メス:20-25g)をそれぞれsham群とくも膜下出血群の2群づつの計4群にランダムに分け、Stroke Therapy Academic Industry Roundtable (STAIR)が提唱する動物実験指針に準じ、評価項目は全て盲検的に検討し、本研究を実施した。くも膜下出血モデルは、臨床例と同様に、くも膜下出血後の頭蓋内圧亢進や脳循環障害、脳血管攣縮に加え、神経細胞のアポトーシスを再現できるくも膜下出血の急性期モデルとして確立している血管内穿通法により作成した。くも膜下出血後24時間、48時間、72時間後に致死率、体重変化率、くも膜下出血の重症度、神経症状、アポトーシスの程度を2重盲検的に評価し、テネイシンC発現抑制によりどのような影響を受けるか検討した。その結果、テネイシンCノックアウトマウスでは神経障害が有意に軽減し、また大脳実質内毛細血管内皮細胞のマイトジェン活性化蛋白質キナーゼの発現を抑制することで、血液脳関門障害を抑制し、脳浮腫を軽減させることを明らかにした。マイトジェン活性化蛋白質キナーゼの発現程度についてはウエスタン・ブロット法にて定量的評価もおこなった。また、ELISA法にて血液中のテネイシンC濃度を経時的に測定し、その濃度がくも膜下出血後のearly brain injuryや遅発性脳障害の発生と相関して上昇する可能性を明らかにした。

当初計画通り、研究を進める予定である。今後は更に、Nを増やし、より研究成果を確かなものとすると共に、テネイシンCが神経細胞のアポトーシスを引き起こす分子機構を解明する予定である。
