| Outline of Annual Research Achievements |
高齢化が進行する本邦においては、心疾患や脳血管疾患などの血管病が、がんに次いで日本人の死因の2位になっている。血管病の1つである胸部大動脈瘤は瘤径が拡大した症例の非手術予後は不良である。一方、手術の重篤な術後合併症として脊髄虚血による対麻痺があり頻度は約5%と高い。回復困難な下肢運動機能低下が生じるため予防が非常に重要となるが、脊髄虚血の機序はまだ未解明であり、従って予防法も不十分である。過去の報告では脊髄虚血モデルラットにおいて、髄液中のグルタミン酸濃度が上昇することが判明している。本研究では脊髄虚血モデルラットを作成し、脊髄前角でマイクロダイアライシス法を用い連続的にグルタミン酸を測定することで脊髄虚血の機序の解明を行い、これに対する治療法の有効性について検討を行う。

脊髄モデルラットの作成を行なった。脊髄前角においてマイクロダイアライシスを行い、脊髄虚血後に脊髄前角のグルタミン酸が直ちに上昇することが判明した。

今後、吸入麻酔薬による虚血プレコンディショニングなどの治療的介入の有効性を検討する予定である。
