Prognostic value of Metabolic Tumor Volume (MTV) in patients with head and neck cancer treated by radiation-based therapy

本研究の目的は、頭頸部癌患者の一次治療前に原発巣と頸部リンパ節転移、および遠隔転移の有無を精査する目的で施行されている18F-FDG-PETおよびFDG-PETとCTとの融合画像であるPET/CTを用いた病巣体積の三次元的計測値であるMetabolic Tumor Volume (MTV) と治療予後との相関を解析することであった。
研究計画について当院の研究倫理委員会による承認を得たのちに診療記録の後ろ向き検討を行った。はじめは当院で加療した頭頸部癌患者全体の検討を予定していたが、原発巣の部位によってMTV値のばらつきが異なるため喉頭癌と下咽頭癌に限った検討を行った。
喉頭癌、下咽頭癌に対して放射線治療を行った場合、いずれの癌種においても治療前のMTV値が高値であった症例は治療後の遺残、再発、長期予後のいずれにおいても予後不良であると示された。また、喉頭癌においてはMTV値が高値である症例に対して手術と術後放射線治療を組み合わせた治療を行うことで放射線治療を行った場合と比較して予後が良好となることが示された。今後は治療前に計測したMTV値による至適治療法選択が可能となりうると考えられた。
上記研究結果につき英論文で発表(PLoS One. 2015 Feb 18;10(2):e0117924、Head Neck. 2016 Nov;38(11):1666-1671、Laryngoscope. 2017 Apr;127(4):862-867)、国際学会での発表(第39回日本頭頸部癌学会 第4回アジア頭頸部癌学会(神戸), 2015年6月)、国内学会シンポジウムでの発表(第28回 日本喉頭科学会総会・学術総会(大阪)，2016年3月)を行った。
