Outline of Final Research Achievements

This study aims to clarify the Seamless care guideline between community medicine and inpatient care performed in psychiatric outpatient department. As a result of analysis, contents of nursing practice in 11 seamless cares were extracted. Academic Significance and Societal Importance of the Research Achievements

精神医療は社会的な動きとして入院医療から地域医療へとその転換が求められているが、長期入院患者の社会復帰や再入院率の高さなど、地域移行・地域定着においては様々な課題がある。本研究により、精神医療における地域医療と入院医療との連携をはかり、スムーズな在宅移行支援、および再発再燃の早期診断・入院治療への移行に貢献できるものと考える。これまで、精神科外来において入院医療との連携については、その重要性が述べられているが、研究の蓄積には至っておらず、これらに取り組むことで精神看護の質向上に貢献できるものと考える。
