イランカスピ海沿岸地域における消化管がんの宿主・環境要因の研究

Research Abstract

イランカスピ海沿岸地域における食道がん、胃がんおよび大腸がんに注目した疫学研究を行ない、当該がんの予防に資する。
まず、平成16年度の初めに、イランカスピ海沿岸において宿主要因および宗教、社会階層などの社会経済的要因、飲酒、喫煙、身体活動などに関する生活習慣調査票および食品リスト、ポーションサイズ、摂取頻度からなる半定量食物摂取頻度調査票を開発した。
平成16年8月に一般住民など130名(男50名、女80名)を対象にして、生活習慣調査票および半定量食物摂取頻度調査票調査を実施し、呼気、血液、尿および便を採集して国際エコロジカル研究を行なった。すなわち、呼気に関してはウレアーゼテスト、血液に関してHp抗体、ペプシノゲン、脂質関連マーカー、抗酸化物質などを分析し、尿は塩分などの排泄量、便はpH、突然変異原性分析、便中金属、胆汁酸分析、腸内菌叢およびHpのPCR分析などを実施し、わが国と当該地区との国際比較を行なった。なお、Hpの遺伝子多型の分析も実施している。
現在、当該地区における食道がんおよび胃がんの症例対照研究を行なっている。採取した血液の血清成分を4分注して、Hp抗体、ペプシノゲン、血清脂質マーカー、脂肪酸濃度・組成分析、抗酸化物質分析などを実施する。白血球層を用いて薬物代謝酵素、アルデヒド脱水素酵素、脂質代謝酵素、サイトカインなどに関する遺伝子多型を解析して、宿主要因の関与を調べる。
1年間に食道がん症例100例(男女合計)、胃がん症例100例、大腸がん症例100例、病院対照群それぞれ100例を目標に症例対照研究を実施しているが、これまで食道がん症例8例(男女合計)、胃がん症例38例、大腸がん症例5例、病院対照群それぞれの病院対照群を収集した。目標対象数の収集を達成すべく働きかけており、研究は順調二進捗している。
なお、平成18年1月には、イランの共同研究者を招聘して、進捗状況、対象数収集の方策などに関する研究打ち合わせを行なった。
