Research Abstract

Patients having very early peripheral lung cancer were investigated. After informed consent was obtained from each of them, thoracic computed tomography was performed, and virtual bronchoscopy was created from the data of the computed tomography. In the console, the route from the trachea to the peripheral shadow was selected, and the route was recorded with the rough sketch. In another day, bronchoscopic examination was conducted to the patient in the computed tomography room. Local and intravenous anesthesia was performed as usual patient.. After usual bronchoscopic examination, approaching to the peripheral shadow was attempted using an ultra-thin bronchofiberscope, with the guidance of virtual bronchoscopy. With the radiologic findings of computed tomography as the reference, transbronchial lung biopsy was performed with an ultra-thin forceps. As the results, accurate diagnosis was obtained in some patients. Using virtual bronchoscopy seemed also useful for decreasing the time for bronchoscopic examination and the time for fluoroscopy. Rough sketch was useful to determine the daughter branches and to refrain from misunderstanding. At present, the computer program for automated selection of the route from trachea to peripheral shadow is being developed.
