リスク提示システムの構築にあたって、本年はいくつかの試験的な解析を行った。まず KiCS レジストリからは、長期的な予後に関する初めての解析を行い、それぞれの心血管イベントの持つ意義を明らかにし（In the KiCS registry, the incidence of a subsequent ACS was associated with higher mortality, but this association was less apparent after unplanned coronary revascularization or unstable angina.[J Am Heart Assoc. 2017 Oct 27;6(11). pii: e006529]）、またこれまで発症の予測が重要とされてきた周術期合併症の費用的な側面の意義の検証も行った（Procedure-related complications, particularly postprocedural AKI, were associated with higher costs in PCI. Further studies are required to evaluate prospectively whether the preventive strategy with a personalized risk stratification for AKI could save costs.[Am Heart J. 2017 Dec;194:61-72.]）。
さらに我が国最大の冠動脈インターベンション登録システムである J-PCI（日本心血管インターベンション学会）のデータベースを用いた解析を行った。その主要な成果としては、個々の患者背景のみならず、施設毎のPCI年間施行実数が非常に短期的な成績に大きな影響を及ぼすことが明らかとなり、本研究結果はより広域なレベルでのフィードバックシステムの開発に結び付くこととなった（In contemporary Japanese PCI practice, lower institutional volume was related inversely to in-hospital outcomes.[JACC Cardiovasc Interv. 2017 May 8;10(9):918-927.]）。

本年もリスク提示に関して複数の学術的な論文を発表することができ、さらにその実証考査に向けての準備も慶應義塾大学循環器内科 ならびに 川崎市立川崎病院にて順調に進んでいる。また、より大きな規模では J-PCI を提供する日本心血管インターベンション学会との連携も深めていき、同学会が提供するフィードバックシステムとも連携を図る方向で作業が進んでおり、昨年12月にワーキンググループが発足した。その成果の一つとして試験的に気象庁の気候データと心筋梗塞の発症に関して解析を行っているが、二つのビッグデータの有機的統合により、主に（相対的・絶対的双方の）気温変化が心筋梗塞の発症に関連することが明らかとなった（Both the absolute value and relative change in the ambient temperature were associated with the occurrence of ST elevation MI after adjustment for these meteorologic variables in Japan.[Am J Cardiol. 2017 Mar 15;119(6):872-880.]）。

| Strategy for Future Research Activity |
今後はさらにビッグデータを用いた個々のPCI周術期合併症の予測の精度を高めるための検証を進めるとと共に、電子カルテのベンダーとの連携を深めていく予定である。本研究に関連している施設が使用している電子カルテが Fujitsu Japanのものであることから、同社の提供する ExChart というプラットフォームに則りアルゴリズムの作成を行っていくこととなっている。
なお、本研究に付随する国際共同研究加速基金を用いた渡航計画に関しては、当初はDuke大学の中央集計センターにて、全国ベースのレジストリデータ集計結果の活用に関して視察を行う予定であった。しかし、本年3月の米国循環器学会での当事業に関するディスカッションを行い、レジストリデータの先駆的な活用に関しては同事業の責任者であったRobert Harrington教授の異動に伴い、Stanford大学に移っているとの助言をDuke大学担当者より受け、変更を申請することとした（本年4月下旬に「渡航先外国機関変更承認申請書（F-17）を提出」）。なお、もう一方の渡航先であるKansas大学については予定通り訪問を行うこととしている。
