筋萎縮性側索硬化症の神経興奮性に着目した革新的治療法開発

Outline of Annual Research Achievements

筋萎縮性側索硬化症(ALS)患者を対象として、ナトリウムチャネルブロッカーの運動神経過剰興奮性抑制作用、進行抑制作用を検証するための臨床試験を計画している。当初、塩酸メキシレチンを用いた臨床試験を計画していたが、その後ラコサミドが2016年7月に製造販売承認を取得したため、これを用いた臨床試験に計画変更を行った。具体的にはALS患者30名を対象に、ラコサミド400mgを内服させ、神経興奮性に及ぼす効果を検討する臨床試験を計画している。プロトコール評価部会の承認が得られたため、治験審査委員会への申請を現在行っている。
その他、ALS末梢神経過剰興奮性がALS臨床機能スケールの低下に深くかかわっていることを、Neurology and Clinical Neuroscience誌に発表した。更に、ALS中枢運動神経興奮性と予後との関係を検討し、中枢運動神経興奮性が高い患者の方が予後不良であることをNeurology誌に報告した。また、ALS末梢および中枢運動神経興奮性と病期の関わりについて検討し、PLoS One誌およびClinical Neurophysiology誌にそれぞれ公表行った。加えて、ALS運動神経興奮性の観点から見た治療戦略に関する論文が、Expert Review of Neurotherapeutics誌に総説として掲載された。
中枢運動神経興奮性の基盤的研究も行い、閾値追跡法2連発経頭蓋磁気刺激検査を用いて、研究成果を公表した。具体的には、健常者の運動神経興奮性の左右差および年齢による変化を明らかとし、Muscle & Nerve誌、Clinical Neurophysiology誌にそれぞれ発表した。
