Outline of Annual Research Achievements

慢性腎臓病(CKD)のマススクリーニングに関する経済評価として、特定健診・特定保健指導の文脈のもとでの腎機能検査の必須項目の変更という政策決定に関する費用効果分析を実施している。初年度からの計画として、経済モデルの構築に必要な臨床エビデンスの2010年以降の新しいデータのレビューを行いつつ、研究代表者がステアリングコミッティー委員を務めている厚生労働科学研究特定健診コホート等の解析を通じた臨床エビデンスの創出を行ってきていた。今年度は、特定健診において尿潜血を追加して行っている保険者で全死因死亡リスクが低下しているという画期的なエビデンスを明らかにすることができた(Iseki K, Konta T, Asahi K, Yamagata K, Fujimoto S, Tsuruya K, Narita I, Kasahara M, Shibagaki Y, Moriyama T, Kondo M, Iseki C, Watanabe T. [Epub ahead of print])。そこで、マススクリーニングのための腎機能検査項目として、尿潜血検査を追加するという政策決定に関する経済モデルの構築を目標として定め、腎臓内科領域疾患（腎炎、IgA腎症）と泌尿器科領域疾患（尿路結石、膀胱癌）を対象疾患とし、膀胱癌部分のモデルの構築まで進められた。

Reason

保健医療プログラムの経済評価を行う前提として、臨床的なエビデンスが確立していなければならない。特にマススクリーニングを対象とする場合には、死亡の削減のエビデンスが確立していることが必要不可欠である。この臨床的なエビデンスを確立できたことによって、本研究の経済評価の政策学的な基盤が確立できた。さらに具体的な経済モデルの構築においては、腎臓内科領域疾患部分と泌尿器科疾患領域部分の両方に取り組むことが課題になったが、泌尿器科領域疾患部分で主たる膀胱癌部分のモデルが構築できたため。

Strategy for Future Research Activity

具体的な経済モデルの構築、精緻化を進める。泌尿器科領域疾患部分については、尿路結石、腎臓内科領域疾患部分については腎炎、IgA腎症のモデルを構築し、全体としてひとつのモデルにまとめ、マススクリーニングのための腎機能検査項目として尿潜血検査を追加するという政策決定に伴う費用対効果を明らかにする。
