Outline of Annual Research Achievements

Genome-wide association study (GWAS)の隆盛により，炎症性腸疾患(inflammatory bowel disease IBD)の疾患感受性遺伝子は163にも及ぶ.しかし，同定された遺伝子の多くは，オッズ比が小さく，クローン病の遺伝素因のおよそ20%を説明し得るに過ぎない.欧米人と日本人の疾患感受性遺伝子には驚くべき相違が認められるが，これまで日本人IBDに関するGWAS は4試験のみであり，さらに，われわれが施行したメタ解析で再現性を認めた日本人に特有な SNPはごくわずかであった.このことから、個々のIBD患者発症に関連する遺伝子を同定するためには、家系内での遺伝子解析、あるいは細胞に機能変化を生じさせるエピゲノム変化をとらえる必要があると考えている。
こうした現状を踏まえ，まず我々は、日本人IBDの小家系におけるパーソナルゲノミクスを解析にとりくんできた. これまでの研究手法を用いて，数家系のトリ オおよび罹患同胞対のexome解析を行い，日本人に特有なIBD関連 single nucleotide variant (SNV)を同定することを試みてきた。我々はすでに3家系(IBD患 者)の解析を終了した。一家系については、Th2サイトカイン(IL-5, IL-13)発現に関連するプロモーター領域の遺伝子多型が認められた。他の家系ではIL-1 betaの発現に関連するプロモーター領域の遺伝子多型が認められた。しかしながら、これらの希少変異の再現性を検討するためには，多数の症例の集積が極めて重要である.したがって、共同研究施設でのさらなる症例集積が必要である。
