肝細胞癌(HCC)は本邦の悪性新生物による死因の第４位を占める疾患であり、その大半はC型肝炎ウイルス(HCV) が70％、約10%がB型肝炎ウイルス(HBV)感染に起因していた。近年、副作用が少ない抗肝炎ウイルス剤の内服が使用可能となり、2015年からは1日1錠12週間で99%ウイルス排除可能な薬剤が認可となった。(Mizokami M, Korenaga M et al. Lancet Infect Dis. 2015) HCVがcontrol可能となりHCV関連HCCは減少すると予想される。一方、非肝炎ウイルス感染者からの肝発癌も増加が確認され、疫学的コホートからは、非肝炎ウイルス感染発癌者の多くが肥満、糖尿病といった代謝異常を合併していることからも、多くの糖尿病合併症例の中から肝病態進行例を抽出すること、またその治療戦略を確立することは急務である。本年度は、発癌59例と非発癌330例の比較検討で、NASH線維化に関与するSNPであるPNPLA3 遺伝子が独立して肝発がんに関与すること、PNPLA3発癌risk alleleを持つ115例の肝発がんに2型糖尿病関連SNPであるJAZF1遺伝子が独立して肝発がんに関与することを明らかにした(Ueyama M, Nishida N, Korenaga M, et al, J Gastroenterol 2016)。また、非侵襲的肝線維化診断法であるFibroScanを用いても、肥満・糖尿病症例では、その有効性が低下し様々な組み合わせを用いて診断しなけばならないこと（Kumagai E, Korenaga K,Korenaga M, et al, J Gastroenterol 2016)や、その他の補助マーカー候補も併せて報告した(Sci Rep 2016) 更に、M2BPGi性(Liu J et al.Sci Rep 2017),WFA-sialylated MUC1(Sci Rep 2017)の発がんへの有効性も報告した。
