Outline of Final Research Achievements

Valvular heart disease registry with aortic and mitral valve disease was established (Tokai Valve Registry). Substantial number of patients underwent transcatheter aortic valve implantation (TAVI) and transcatheter mitral valve repair with MitraClip. Treatment trends for elderly patients with significant valve disease were revealed.
We utilize intracardiac echocardiography via jugular vein which enables conscious sedation, minimal invasive TAVI. It is safe and effective strategy which we have reported in international cardiology congress. We also have published a paper regarding the usage of self-expandable transcatheter heart valve system in order to reduce new onset conduction disturbance which is still one of the Achilles' heels of TAVI.

Academic Significance and Societal Importance of the Research Achievements

人口の急速な高齢化に伴い、大動脈弁狭窄症や僧帽弁閉鎖不全症の有病率は上昇しており、重症度が中等度以上の弁膜症は予後を悪化させることがこれまでの疫学研究で証明されている。本邦においても、同様の傾向が認められており、どういった患者が低侵襲カテーテル手術の恩恵を受けることができるか、明らかにすることは、急増する医療費抑制にもつながる可能性のある重要な課題である。
今回、我々が構築した弁膜症レジストリー(Tokai Valve Registry)により、上記課題解決につながることが期待され、また同治療の安全な普及に役立つと考えられる。
