Pathophysiological effect of fatty acid composition in pulmonary surfactant on lung diseases

Outline of Annual Research Achievements

我々はこれまで、飽和・不飽和脂肪酸の鎖長を伸長させる酵素Elongation of long chain fatty acid 6(Elovl6)がⅡ型肺胞上皮細胞に発現し、特発性肺線維症患者ではこのElovl6の発現が著明に減弱していること、またElovl6欠損マウスにブレオマイシンの気管内投与を行うと、肺組織内で飽和脂肪酸であるパルミチン酸が過剰に増加し、酸化ストレスやアポトーシスが亢進して、肺線維症が増悪することを明らかにした[Sunaga H, et al. Nat. Commun. 2013]。
さらに、野生型およびElovl6欠損マウスに喫煙曝露肺気腫モデルを作成したところ、Elovl6欠損マウスでは肺胞腔の拡大、弾性線維の破壊などが顕著で、肺気腫様の病変が増悪していた。両群における脂肪酸組成を比較したところ、Elovl6欠損マウスでパルミチン酸分画の減少、オレイン酸分画の増加を認めた。さらに喫煙曝露Elovl6欠損マウスでは、肺サーファクタントに必要なホスファチジルコリン生成に関わる酵素、LPCAT1、LPCAT4の発現が著明に低下していることを見出した。また、LPCAT1、LPCAT4の発現低下に伴い、肺傷害が増悪することが知られ、本研究でもこれらが肺気腫病変の増悪に寄与していることが示唆された。
本研究成果から、我々は肺における飽和・不飽和脂肪酸組成の不均衡が肺線維症や肺気腫の病態形成に重要な役割をしており、その組成を調節しているElovl6は呼吸器疾患の重要な標的因子であることを明らかにした。今後は、脂肪酸組成の変化に伴う肺サーファクタントの量的・質的な変化と肺線維症・肺気腫との関わりや、Elovl6の発現や活性を制御する食事成分、薬剤の解明など、肺疾患予防・治療の新しいアプローチの可能性を検討していきたい。
