Outline of Annual Research Achievements

悪性リンパ腫細胞でのHLA発現、NFkB関連遺伝子の異常と周囲、末梢血のリンパ球数、浸潤細胞の種類を検討した。144例のR-CHOP療法を施行されたDLBCLで検討した。多変量解析でHLA-Class II 発現の低いことは予後不良と抽出された。一方でHLA Class I発現の低いことは単独では予後不良因子とはならなかった。Rituximabの併用によりClass I 陰性であってもADCCによって細胞障害を受けるためと考えると説明可能であった。
しかし，末梢血リンパ球数，CD3数が少ない場合には、発現が高いことが予後不良因子となっていた。NK細胞による細胞障害効果の有無が予後に関係する可能性が有り，リンパ球が少ないとClass Iの陽性細胞でNK細胞による免疫細胞からの認識がなくなってしまい予後が逆転したのであろう。
Class IITA遺伝子あるいは，PDL1, L2遺伝子の発現及び，転座dataの有無をホジキンリンパ腫、縦隔B細胞リンパ腫，その中間型灰色リンパ腫で検討した。既報と異なり日本人ではClass IIITA遺伝子の転座による失活は少なく、ホジキンリンパ腫では，PDL1の発現が高久、縦隔B細胞リンパ腫ではPDL2が高く，PDL1, L2の発現は相互排他的であった。灰色リンパ腫では，縦隔B細胞リンパ腫のパターンであった。ホジキンリンパ腫ではその半数が陰性調節遺伝子である TNFAIP3遺伝子が変異あるいは，欠失を生じている。周囲の浸潤細胞との関係ではCD25遺陽性細胞でみたTreg細胞，CD68圧砕棒陽性細胞，PD1陽性細胞はそれぞれ，半数のホジキンリンパ腫で認めたがこれらの関係ははっきりしなかった。

Reason

臨床検体とその臨床情報の収集を行い、1999年以降，現在まで国立がん研究センター中央病院で悪性リンパ腫と診断された患者のうち核酸抽出が可能な十分量の凍結細胞あるいは，ホルマリン固定パラフィン包埋標本が保存されている患者を対象として，診断のために採取された検体から日常診療に供された後の残余FFPEまたは，凍結組織標本が核酸抽出に用いられる検体を収集した。
免疫染色，FISH法、FICTION法を行い、NFkB関連遺伝子のA20, Blimp1遺伝子の欠失有無とヒストンリジン脱メチル化酵素であるJMJD2C遺伝子の増幅の有無を調べる。ホジキンリンパ腫は50例、縦隔原発びまん性大細胞型B細胞リンパ腫では30例程度である。p50, p52, p65, RelB, c-Relの核内発現の有無は未染である。

Strategy for Future Research Activity

縦隔原発びまん性大細胞型B細胞リンパ腫,は、A20, CARD11, MyD88, CD79a/b, Blimp1などのNFκB関連遺伝子とエピジェネティクス関連遺伝子のMLL2, E2HA, CEREBBP, EP300変異を解析するターゲットシークエンスのパネルを作成する。
次世代シークエンス解析にはLife technology社のIon Torrentシステムを使用する。検体から抽出したDNAを用いてライブラリーを作製する。症例ごとにエマルジョンPCRでタグ付けを行い、マイクロチップを用いたシークエンス反応をする。各遺伝子で変異や融合を検出した検体に関しては、変異・融合検出の感度・正確性を把握するため、サンガーシークエンスによりコール結果を確認する。

次年度中に代表研究者が異動することになり，研究設備のセットアップなどの経費が必要になることが予想されたため，次年度に一部を回した。
