Outline of Annual Research Achievements

本研究は小児先天性心疾患に対する外科治療において肺動脈閉鎖症や主要体動脈肺動脈側副血行など肺動脈低形成を伴う疾患群に対して段階的肺動脈形成手術を行う際、生体内組織形成技術に基づき患者自身の体内で作成した膜状の自己結合組織を血管壁素材として狭小化した肺動脈の拡大形成術を試みるものである。自己結合組織膜は動物実験においては血管壁素材として自家移植を行った場合の安全性や成長の可能性、耐圧性能等についてすでに立証されているが本研究の最終的な目標は同様の安全性や成長可能性がヒトにおいても成立することを臨床的に確認することである。最終年度に1名、研究期間全体において5名の患者(いずれも肺動脈閉鎖症、主要体動脈肺動脈側副血行の疾患群に対して肺動脈統合化手術または肺動脈形成術を施行)にシリコン基材の植え込みを行った。うち2名は次期手術時にシリコン基材周囲に形成された自己結合組織膜を採取後、再度植え込みを行った。胸骨正中創内またはシリコン基材採取後の皮膚切開部から上腹部の肋骨弓下、皮下組織層に外径6.3mm、長さ40-50mmのシリコン基材を各2-4本ずつ植え込んだ。5名のうち3名は6ヶ月-34ヶ月の体内作成期間を経て次期手術(1名は二心室修復術)時にシリコン基材とともに周囲に形成された自己結合組織膜を採取して肺動脈拡大形成を行った。いずれも自己結合組織膜の生成は良好で肺動脈拡大形成術を行う際に問題は認めなかった。また術後経過の観察においても形成部の瘤化などの変化は認めていない。
