analysis on genetic abnormality related to aggressive behavior of uterine leiomyosarcoma

Outline of Annual Research Achievements

すでに子宮平滑筋肉腫と診断されて治療が行われている、ホルマリン固定パラフィン包埋検体を用いた解析を実施した。病理学的に子宮平滑筋肉腫と診断された摘出子宮検体から、それぞれ肉腫組織と（正常）筋層組織を採取し、それらからmRNAを抽出した。抽出したmRNAのqualityを確認後、デジタルカウント遺伝子発現解析（ncounter）およびアジレント社のSurePrint G3Human GE 8x60K v3を用いた網羅的遺伝子発現解析を実施した。前者においては、正常筋層組織と比べて肉腫組織内の免疫環境の相違を検討すべくPanCancerImmune Profiling for Humanパネルを用いて検討した。その結果、正常筋層に比べた場合、肉腫組織においては免疫細胞の数が全体として減少している傾向を示すことが確認された。
さらに、子宮に限局した子宮平滑筋肉腫症例6例において腫瘍免疫と予後との関連を検討した。治療はいずれも子宮及び両側付属器切除術および術後補助化学療法を施行し、6例中4例が再発し原病死された。再発まで12ヶ月以上の予後良好群とそれ未満の予後不良群に分けて検討した結果、予後良好群では特にT細胞免疫系の亢進が確認された。この結果は網羅的遺伝子発現解析の結果とも一致していた。すなわち、腫瘍浸潤リンパ球は重要な予後因子であり、個々の腫瘍の分子生物学的な特徴と増殖過程で形成される微小環境との関連を検討し、その最適化を行うことが子宮平滑筋肉腫患者の予後を改善する方法になるものと推察された。
