Outline of Annual Research Achievements

本研究は、難治性かつ希少がんである骨巨細胞腫(Giant cell tumor of bone: GCTB)の治療成績の改善を目指し、その要となる腫瘍悪性度・治療抵抗性の機序について特定し、悪性度(再発・転移)・治療奏功性に関するバイオマーカーの同定及び、新規治療分子標的の開発を進めた。特に近年GCTBの新規治療薬であるDenosumab(RANKLを標的としたヒト型モノクローナル抗体製剤・分子標的治療薬)が開発され、作用機序(通常、receptor activator of nuclear factor κB ligand(RANKL)は破骨細胞の分化・成熟・機能および生存を制御して作用しているが、そのRANKLとDenosumabが結合することにより破骨細胞へのシグナル伝達を抑制し骨溶解を防ぐ)は骨巨細胞腫においてはまだ不明な部分も多い。そのため、 (1) GCTBの悪性度及びDenosumab治療反応性に関わる発現データベースの構築(主にタンパク質発現)の基 に、(2)それらで同定されたGCTBの悪性度及び治療反応性に関わる分子生物学的機能解析やネットワークの解明を進め、(3)バイオマーカーの同定や新規治療分子標的の開発を大規模検証の基に進めた。具体的には、GCTBの悪性度及びDenosumab治療反応性に関わる生物学的な特徴をプロファ イリング・データベース構築を行うために、12ペアのDenosumab治療前・治療後凍結手術検体セット収集を行った。その手術検体を用いて網羅的なタンパク質発現をiTRAQ(Isobaric Tag for Relative and Absolute Quantitation)の手法を用い研究を進め、特にDenosumab使用における、組織学的硬化(骨化)に関連するタンパク質発現データベース構築に成功した。それらデータベースはネットワーク解析も行い、RANKL経路との関係解明も行った。同定された因子は免疫化学染色を行い、GCTBのアーカイブ検証セットにて発現検証と臨床病理学的因子との相関を解析した。
