Outline of Annual Research Achievements

皮膚疾患は、特殊で高価な機器に頼る検査ではなく、皮膚症状そのものの観察がその診断のために最重要である。また、「皮膚に異常がある」という観察は、特に専門的な知識を持たずとも可能である。他の疾患よりも簡便な方法で探知あるいは診断できるにも関わらず、アフリカにおける皮膚疾患を対象とした体系だった統計は未だとられておらず、その分布およびdisease burdenについて不明である。本研究では、特に世界的な疾病対策対象疾患である「顧みられない熱帯病 neglected tropical diseases (以下、NTDs)」に重点をおき、アフリカにおいて①学童皮膚検診、②スマートフォンを用いた遠隔皮膚診療teledermatologyのシステム構築を行い、その皮膚疾患の実態を明らかすることを目的としている。さらに、２つの介入法の疾病対策としての有用性の検証を目的としている。
H29年度は、西アフリカ・コートジボワールのGagnoa地域の3つの村において、学童皮膚検診と集団皮膚検診を通して、ハンセン病やブルーリ潰瘍といった皮膚症状を特徴とするNTDsを診断した。H30年度は、スマートフォンによる経過観察、および、必要時に遠隔診療を実施した。さらに、地域看護師による地域住民への定期的な啓発活動を通して、NTDs症例を受動受診で診断に至ることができ、複数のアプローチから成るNTDsのための新規疾病対策構築を達成することができた。
学童の時点で早期診断・早期治療することは、NTDsの最大の問題である後遺症率を低下させることが可能となる。結果、個々の患者の「生活の質」とともに「日常生活動作」を向上し、彼らの能力を発揮できる将来的基盤を築くことが本研究の長期アウトカムであり、今回の成果がより広い地域に広まることで達成が期待できる。
