大脳皮質体性感覚野における痛覚情報処理機構の解明

Research Abstract

大脳皮質体性感覚野における痛覚情報処理機構を明らかにするため、shamラットと慢性炎症ラットを用い、in vivoパッチクランプ記録法によって後肢皮膚へ加えた機械的および侵害性温度刺激誘起のシナプス応答を解析した。体性感覚野深層の錐体細胞から記録を行うと、記録を行なった全ての細胞は自発性のbursting活動を示し、高振幅のburstingには活動電位が重畳していた。Burstingの発生頻度および振幅、また、活動電位の振幅や持続時間にはshamと炎症ラットで有意差はみられなかった。次に触刺激による応答を解析したが、burstingおよび活動電位の発生頻度には変化がなかった。そこで、鈎付きのピンセットを用い、皮膚への機械的侵害刺激に対する応答を解析した。記録細胞の受容野に痛み刺激を加えると、shamラットでは30%の細胞で活動電位の発生頻度が増加したが、炎症ラットでは約60%に増加した。また、shamと比較し炎症ラットでは活動電位の発生頻度が高く、膜の脱分極を伴っていた。活動電位の発生頻度はshamに比べ有意に炎症ラットで高かった。膜の脱分極は刺激後には元に戻り緩徐なシナプス応答は観察されなかった。この結果から、膜の脱分極はEPSPまたはburstingによる加重のためと考えられた。次に、熱刺激を加えたときの活動電位の頻度の変化を解析した。皮膚に50℃の熱刺激を加えると、活動電位の発生頻度が上昇する細胞が観察された。Shamラットでは約30%の細胞で、炎症ラットでは約60%の細胞で観察され、shamラットと比較して有意差があった。熱と同様、アセトンによって冷刺激を加えると、shamでは約20%の細胞で活動電位の頻度が増加したが、炎症ラットでは55%と有意に頻度が増加した。以上の結果から、慢性炎症時においては、機械的侵害刺激や熱および冷刺激の応答で、活動電位を発生する細胞の割合が増加し、かつ活動電位の発生頻度も有意に増加した。しかし、burstingの発生頻度には変化がなく、活動電位の発生にはburstingの振幅の増大を伴っているためか、または、錐体細胞の興奮性が変化したためだと思われるが、今後、この点を明らかにする必要がある。
