1. 【The study of chronic care model and the report of annual meeting of 26^<th>, Japan Academy of Nursing Science】
By the transformation of Disease, persons with chronic illness were over 50% of the US populations
Chronic illness (ex: Diabetes mellitus). it will never curable, so healthy life is impossible.
In Pittsburgh University Diabetes Institute, our research project studied the practice concerning "chronic care model".
In the many care setting `(rural area clinic, public heath center, Medical Center of Pittsburgh University.)
The report of the practical study, we presented in annual meeting of 26^<th> Japan Academy of Nursing Science.

2. The 2^<nd> Symposium of National Standards of Diabetes Self-Management Education
American Diabetes Association reported Position Statement of National Standards for DSME (Diabetes Self Management Education) in 1986. ADA gave the Certification for the adapt institution. But in Japan, National Standards for DSME was not created. But many articles of DSME was published recently, in Japan, the Evidence based DSME has been creating in the many care setting,.
3) 【Decide of Guidelines of Diabetes Self-Management Education】
Our research project studied the previous Clinical Guidelines and selected the contents of DSME Guidelines.
And we recognized the purpose of the "National Standards " and "Guidelines".
Our research group decided on the Recognized diabetes educational programs and effective intervention.,
