Research Abstract |
In our institute, angiogenetic therapy with autologous bone marrow transplantation had been performed with permission of Institutional Review Board. We have been examining patients with ischemic heart disease who were possibly treated with autologous bone marrow transplantation combined with coronary artery bypass surgery. We had examined candidates serially, and followed up after treatment. We performed angiogenetic therapy in 2 patients who had severe ischemic myocardial regions where coronary bypass or percutaneous coronary intervention were inappropriate for the treatment. Forty patients who had percutaneous coronary intervention served as control. Standard echocardiographic and tissue Doppler echocardiography were performed before and 6 to 9 month after treatment. There were no complications during angiogenetic therapy. We examined longitudinal LV function (myocardial Doppler strain). In patients with angiogenetic therapy, thallium myocardial scintigraphy was performed to evaluate the improvement of myocardial perfusion after the therapy. Myocardial perfusion at the regions treated with bone marrow transplantation had improved significantly in 2 patients. Contrary to the finding, Myocardial longitudinal systolic function assessed by strain Doppler imaging had not shown significant improvement at the regions treated with angiogenetic therapy. In control patients, radial myocardial strain in ischemic area was improved after treatment with percutaneous intervention, but the area with old myocardial infarction or non-treated region with normal systolic function had not shown any changes in tissue Doppler measurements. From these results, it is possible that angiogenetic therapy with autologous bone marrow transplantation improves myocardial perfusion, but does not improve regional wall motion.
