Research Abstract

パーキンソン病の自律神経障害が近年QOL(生活の質)の観点から注目されている。このうち排尿障害(とくに頻尿・尿失禁)は約70%と高頻度にみられ、患者のQOLを損なうことから、治療上の重要なターゲットと考えられる。近年の研究によれば、排尿機能は、単純な脊髄反射ではなく、複雑な中枢神経支配を受けるとされている。すなわち、パーキンソン病における排尿障害の解明のためには、排尿に関連した中枢神経系の研究が重要と考えられる。
2005-2008年度我々は、オス猫15頭を用いて、ケタミン麻酔下に挿管・呼吸器を装着、CO2モニター・動静脈カテーテル・膀胱カテーテル・外括約筋筋電図電極を装着し、自発性排尿反射を誘発。定位脳固定装置(Narishige)上でtungsten電極を線条体(尾状核、被殻)に刺入。同部位で単一神経活動電位記録・微小電気刺激を行った。微小電気刺激の結果、排尿抑制部位が尾状核腹側部に見出され、同部位に、排尿反射に応じて発火する単一ニューロン活動が認められた。すなわち、線条体は排尿の高次制御に積極的にかかわっている可能性があると考えられた。
