| Outline of Annual Research Achievements |
CRPCを含む進行前立腺がん86例へ、治療経過で継時的にCTC解析を行なった。CTC回収方法は、抗体蛍光染色によるCytoQuest(Abnova社)、抗体磁気ビーズによるAdnaTest(Qiagen社)を用いた。AR-V7(androgen receptor splicing variant 7)の有無やAR変異の有無、PSMA(prostate-specific membrane antigen)などの前立腺がん関連発現分子の発現を主に解析した。86例から延べ267回のCTC検出を試み、検出率は66.1%(154/267)であった。CTC検出可能であった患者は、採取時の腫瘍マーカーPSA値が有意に高かった(p < 0.001)。AR-V7発現率は19%(14/72人)で、AR-V7陰性CRPCに対する新規内分泌治療薬の奏効率は87%(13/15)であった。また、CTCにおけるPSMAの発現は62％であり、PSMA, AR, AR-V7 およびEGFR発現は治療の2nd-line以降の患者、つまりより進行している患者で有意に陽性率が高いという結果になっ(p<0.05)。 臨床的に治療を変更した56人の症例のうち、39例でCTC検出でき、またそのうち20例でPSMAが陽性であった。PSMA発現は、治療によるPSA減少と有意に逆相関していた。
今後のPSMAの機序の解明として、in vitroの系でPSMAの機能解析を行っている。現在、PSMA発現が陰性の前立腺がん細胞株にPSMA発現ベクターをtransfectionして安定発現株を作成している。
