Outline of Annual Research Achievements

WTマウスおよびCaM HAマウスにおけるMI作成後の解析: WTマウス、CaM HAマウスに対してLADの結紮によるMI作成を行う。MI作成4、8週間後に心エコーにて左室径・壁厚、fractional shorteningを計測した後、エピネフリン/運動負荷による不整脈誘発試験を行う。さらに、心筋細胞を単離培養し、①Ca2+ transient、SR Ca2+ content、Ca2+ spark、cell shortening;②anti-CaM, anti-GRK5, anti-HDAC4,5, anti-p-HDAC4,5の抗体を用いた免疫染色によるCaM, HDAC, GRK5の核・細胞質内分布、リン酸化HDACの割合および心肥大関連遺伝子(c-fos, c-jun)の発現;③RyR2に対するCaM結合能などを測定する。その結果、WTマウスに対するダントロレン慢性投与群、CaM HAモデル群ともに、MI作成後にみられるカテコラミン負荷誘発性の心室頻拍が抑制され、左室リモデリング、心筋細胞肥大も抑制され、心筋細胞内Ca2+ sparkの増大(Ca2+漏出)、cell shorteningの低下、Ca2+ transientの低下も抑制され、予後は著明に改善した。
GRK5のknock-downによる心肥大抑制効果の解析:マウスの新生児培養心筋細胞を用いて、siRNAを用いたGRK5のknock-downを行い、エンドセリン(ET)による刺激でみられるCaMの核内移行と心筋肥大が、抑制されるかを観察する。その結果GRK5のknock-downはCaMの核内移行を抑制することが確認された。
臨床研究アーム:ダントロレンの心不全・重症不整脈に対する有効性を証明する多施設共同二重盲検ランダム化比較試験を開始した。

Reason

CaM HAマウスにおけるMI作成後の解析が予定通り進んでいる。さらにGRK5のknock-downも肥大抑制効果を示した。臨床研究アームにおいてもダントロレンの心不全・重症不整脈に対する有効性を証明する多施設共同二重盲検ランダム化比較試験を開始するなど、ほぼ計画通りに進んでいる。

Strategy for Future Research Activity

今後変異GRK5(GRK5内にある核局在シグナルのdeletion、CaM結合ドメインのdeletion)のトランスフェクションを行いET刺激後にCaMの核内移行、心筋細胞肥大が抑制されるかを観察する。
ダントロレンの心不全・重症不整脈に対する有効性を証明する多施設共同二重盲検ランダム化比較試験(RCT)の症例数を増やす。
