| Outline of Annual Research Achievements |
1.腸内フローラのゲノム網羅的な解析
MHC不適合のB6→BDF1モデルを用い腸内細菌叢のメタゲノム解析を行った。GVHD非発症マウスでは有意な変化を認めなかったが、GVHD発症マウスではdysbiosisが認められ菌多様性低下が確認された。大腸菌やBacteroidesの腸内細菌叢に占める割合はGVHDの重症度正の相関関係を、Lactobacillusの割合は負の相関を示した。これらの結果からGVHDの増悪や軽快にこれらの腸内細菌が関与している可能性が示唆された。
2.Dysbiosis予防・治療の試み
MHC不適合のB6→BDF1モデルにおいて、Paneth細胞増殖効果を有するR-Spo1と遺伝子組換えdefensin4 (Crp4)を投与し、GVHDに伴うdysbiosis予防効果を検討した。R-Spo1はPaneth細胞増殖、defensinの分泌効果を示し移植後のdysbiosisを有意に抑制した。またCrp4の投与によっても，移植後のdysbiosisが抑制され，その結果，GVHDの重症度も改善した。Defensinは移植後においても腸内細菌叢の構成を大きく変化させることなく病原菌増殖を抑制し、GVHDを軽症化できることを見出した。
3.臨床サンプルを用いた腸内フローラの網羅的解析
国際共同研究プロトコールに基づき，造血幹細胞移植症例の移植前，移植後7日目，21日目，35日目，49日目，63日目，90日目，180日目，360日目の糞便を採取し、凍結保存の上米国に空輸し，メタゲノム解析を施行する研究を開始した。すでに検体の一部は米国に空輸しシークエンスが終了している。今後も解析サンプルの米国への空輸を継続して行い，シークエンスが終了したサンプルにおいては臨床データとの照合を行う予定である。

研究３項目すべてにおいて進展がみられた。腸内フローラのゲノム網羅的解析では移植成績に関連する複数の細菌群が同定でき、他のグループからの報告と同様であり信頼できる解析結果と考えられた。遺伝子組み換えalpha-defensin投与による細菌叢の正常化はいわば世界初の"proof of concept"となった点で価値が高い。また臨床サンプルの解析は国際共同研究であり、検体の精度管理と輸出体制を整備できた点が高く評価できる。

| Strategy for Future Research Activity |
小腸Paneth細胞の解析がほぼ完成できたことから、今後は大腸における重要な分泌細胞であるムチン産生Goblet細胞の機能解析と造血幹細胞移植後の動態解析を進める予定である。またPaneth細胞増殖因子の一つとしてR-Spondinが同定されたが、その産生細胞は不明であり、その同定を目指す。また臨床サンプル解析では国際共同研究以外に、独自研究として糞便サンプルにおいてdefensin濃度とdysbiosisが臨床サンプルにおいても関連するかを評価し、血清サンプルではdefensin濃度がGVHD発症を予測するためのバイオマーカーとして機能するか否かを評価する。
