Outline of Annual Research Achievements

従来,偏光感受型光干渉断層計(Optical Coherence Tomography, OCT)による生体計測は反射信号による評価や,複屈折の擬似カラー化による質的評価が行われてきたのみで,定量的な指標は存在しなかった.本年度我々は,phase retardationを数値化し,組織の質の評価をさらに精緻化する試みを行った.線維柱帯切除後の濾過胞を従来のOCTと偏光OCT同時に5ライン測定する.得られた画像をImageJソフト(National Institutes of Health, MD)で解析し,phase retardation valueを算出する.また,濾過胞に様々な画像処理を施すことにより,fibrosis scoreを算出.これらのパラメータと,従来のOCTで評価される濾過胞内部形状に関する因子や,眼圧や点眼薬種類などの臨床因子との関連を,多数例にて解析した.phase retardation valueやfibrosis scoreの臨床的意義,正当性を評価した.
これらの評価を正常眼および緑内障術後眼のみならず,円錐角膜眼でも行い,定量的指標の妥当性について検討した.概ね,臨床的な重症度や病気進行程度を良く反映したものであり,絶対値の正当性はまだ確立していないものの,相対値としては有用な指標が得られていると考えられた.

装置のキャリブレーションを見直すことにより29年度中の遅れを取り戻し,当初の計画通り生体眼における評価を進めることができている.

Strategy for Future Research Activity

特に問題点はなく,臨床症例の蓄積,解析方法の確立,正常眼のみならず病的眼における測定値の妥当性を検討していく予定である.
