Development of a 3D dynamic myocardial phantom toward standardization of myocardial gated SPECT image

Outline of Annual Research Achievements

本研究の目的は,心電図同期心筋SPECT評価用三次元心臓動態ファントムをより人体類似構造となるよう改良し,本ファントムの実用性を検証することである。本年度研究では,1)正常心筋モデルによる心拍数および左室容積および左室駆出率など心機能評価指標の検証,2)病態モデルとして,拡張型心筋モデルおよび肥大型心筋モデルを作成し,そのモデルの妥当性と心機能評価の検証,さらに,3)多施設(複数装置)の心機能精度管理用ファントムの実用性として,各装置の心電図同期心筋SPECT 検査の臨床撮像条件で本ファントムを収集し,画像解析プログラムで本ファントムの心筋血流画像および心機能指標値を検証する。
その結果,正常心筋モデル(左室容積:ESV 40.0 mL,EDV 110.4 mL,LVEF 63.8 %)の心拍数を40~100 bpmで変化させた結果,心拍数による収縮の速度,壁厚の変化が確認できた。病態モデルでは,肥大型心筋症モデル(EDV40.9mL,ESV20.8mLおよびLVEF49.2%)とし,心筋を16mmに肥厚させた。拡張型心筋症モデル(EDV252.3mL,ESV 202.0mLおよびLVEF19.9%)を作成した。位相解析の指標値は,正常モデルに比し病態モデルでbandwidth,phaseSDおよびentropyがいずれも低値を示し,その特徴が再現できた。装置間の検討では,Precedence 16とSymbia T6でそれぞれ収集を行った。その結果,理論値と各装置の指標値には全てにおいて有意に強い正の相関が認められた。
心電図同期心筋SPECT評価用三次元心臓動態ファントムを改良することにより,実際の身体の心臓の動きの再現性が高まった。本ファントムを用いた心電図同期心筋SPECTでは臨床で見られる様々な心機能条件の心機能評価を行うことができると考える。
