Outline of Annual Research Achievements

前年度から引き続き、急性非代償性心不全(acute decompensated heart failure:ADHF)において、体液貯留、終夜体液シフトの制御に対する特異的アプローチによって、体液貯留・終夜体液シフトの是正、血中CO2レベルの上昇などを介して中枢性睡眠時無呼吸(central sleep apnea:CSA)の重症度が軽減できるか否かを解明することを目的として、下記の介入試験の被検者登録を行った。
1)ADHF・CSA患者に対する減塩強化に関する無作為化試験：ADHF入院患者のうち、初期治療による状態安定後にベースラインの睡眠ポリグラフ検査(PSG)、終夜体液シフト量の測定、経皮CO2モニタリングなどの各評価を行い、ベースラインPSGでCSAと診断された症例を通常減塩食(食塩6g/日)継続群と強化減塩食(食塩3g/日)群に無作為化に割付し4日後にベースラインで行った検査・測定・データ収集を反復した。現在までに25例を組み入れ解析を行っているが、一部逸脱とみなされた症例があり21例分のデータがある。逸脱を除く症例で24症例まで登録し終了となる予定である。
2)ADHF・CSA患者に対する下肢弾性ストッキング(compression stocking: CS) 着用に関する無作為化試験：ADHF入院患者で退院前にCSAが確認されている症例を対象にして、退院前の段階で、退院後の日中にCSを着用する群と着用しない群に無作為化しフォローアップ評価を行う研究で、臨床研究法施行後の委員会審議受け入れが遅延していたことと、使用予定であったストッキングのモデル変更などで遅延していた。モデル変更後の低圧ストッキングの下肢体液貯留への影響をみるパイロットデータの収集を行い、変更前のストッキングとほぼ同様の効果があることを確認した。
