Outline of Annual Research Achievements

プリオン病は致死性急速進行性認知症であり、プリオン病の診断法・治療法は未だ確立されていない。我々はプリオン病患者髄液中の超微量の異常プリオン蛋白の検出に世界で初めて成功した。本研究は世界に先んじて血液を含んだ非中枢神経の臓器からのプリオン病の革新的なさらに非侵襲的な診断を試みることを研究目的とした。2017年から2021年3月までにこれまでの研究成果を基に４つの研究を基盤に画期的な診断法を確立することを行った。I.微量の異常プリオン蛋白の半定量化の確立、特にヒトサンプルでの微量の異常プリオン蛋白の半定量化を確立した。II.保存しているサンプルでの解析（後向き研究）と4年後前向き試験による研究を行い、髄液から診断に対する感度・特異度を確立し、半定量も行った。III.プリオン病患者のサンプルからの異常プリオン蛋白の新規検出法を開発し、その有効性について検討を行った。IV.プリオン病患者のサンプルからの異常プリオン蛋白を検出できた臓器についてヒト型ノックインマウスで感染実験を行い、感染性について検討した。
2017年と2018年度にヒトサンプルでの微量の異常プリオン蛋白の半定量化を確立し、論文化し投稿した。さらに保存しているヒトサンプルについても定量化し、後向き試験のサンプルの感度・特異度については学会にて発表を行った。
2019年と2020年度は4年間の前向き試験による研究を行い、髄液から診断に対する感度・特異度を確立し、半定量化も行った。過去のホルマリン固定の脳から微量の異常プリオン蛋白の検出及び半定量を行う予定であり、臓器から微量の異常プリオン蛋白の半定量化を行い、論文化し投稿した。さらにプリオン病患者のサンプルからの異常プリオン蛋白を検出できた臓器についてヒト型ノックインマウスで感染実験を行ったがうまくいかなかった。原因として動物と人間の種の違いと寿命の違いと考えられた。
