Outline of Annual Research Achievements

本研究の目的は、重症筋無力症におけるクリーゼの現状の把握と、クリーゼが起きるときにどのようなメカニズムが働いているのかを解明することである。クリーゼは重症筋無力症におけるもっとも重症な形態であり、クリーゼの病態解明をすることは重症筋無力症の増悪の機序を明らかにすることになる。
クリーゼの病態解明の第一段階として、臨床疫学的な側面からみた解析をおこなった。各種の臨床パラメーターとクリーゼ発症との関連を検討するものである。多施設の症例を検討した結果、罹病期間2年以上10年未満の全身型重症筋無力症患者369例を集積し、これをクリーゼのある群45例とクリーゼの既往のない324例とに分けた。アセチルコリンレセプター抗体陽性率はクリーゼ群で高かった。筋特異的チロシンキナーゼ抗体陽性率はクリーゼ群で圧倒的に高かった。早期速効性治療（クリーゼに対する処置以外）の実施率はクリーゼ群で低かった。このことは、早期速効性治療がクリーゼの抑制に寄与していることが示唆している。
クリーゼの病態解明の第二段階として、統計学的手法を用いてクリーゼのリスクファクターの同定をこころみた。10件のcase-control研究、および多施設研究のデータをもちいてメタアナリシスとロジスティック回帰分析をおこなった。その結果、case-control研究では球症状、クリーゼの既往、疾患の重症度、肺活量の低下がリスクファクターとしてあがったが、多施設研究の解析では最重症時の球症状QMGがOR2.02と最大のリスクファクターであることが示された。
第三段階として、メッセンジャーRNAの網羅的解析をおこなう予定であったが、新型コロナウイルス感染症の影響で研究がおおきな打撃を受け、そこまで解析ができなかった。
