Outline of Annual Research Achievements

急性リンパ性白血病(ALL)の診療において、クローナルな免疫グロブリン(Ig)・T 細胞受容体(TCR)遺伝子の再構成の検出は微小残存病変(MRD)の評価などにおいて不可欠である。1歳以下に発症する乳児ALLは、難治例が多く、かつIg/TCR再構成を用いたMRD評価が困難であることが知られている。そこで、乳児ALLの臨床検体を対象に、昨年度までに確立したターゲットキャプチャーシーケンスを用いたIg/TCR再構成の同定を行った。乳児ALLはKMT2A(MLL)転座例が多く、MLL転座例とMLL野生型例に大別されるが、MLL野生型例に観察される再構成は、1歳以降に発症したB細胞性ALLと類似するものであったが、多くのMLL転座例では特徴的な再構成パターンが観察された。すなわち、MLL転座例においては、IGH以外の遺伝子に再構成が認められる症例は限られ、IGH再構成も完全なVDJ再構成ではなく、DJ再構成が多くの症例で認められ、MLL-AF4転座例で顕著であった。一方で、1歳以降に発症したB細胞性ALLに比して、1例あたり同定されるIGH再構成の数は多く、オリゴクローナルな病態が多いことが類推された。以上より、MLL転座例では、未分化な細胞を起源に、未分化な状態で腫瘍化していると考えられた。IGH再構成においては、再構成が生じるVDJ領域が、VH6-1やDH3-9など、限られた領域で生じていることも明らかとなった。限られた領域で再構成を起こしていることにより、従来のmultiplex-PCRに基づく再構成の同定が難しい理由となっていると推測される。ターゲットキャプチャーシーケンスの導入によりクローナルな再構成の検出率は大きく改善したが、オリゴクローナルな病態があることを考えると、Ig/TCR再構成に基づく、正確なMRD評価は難しい病態であることが推測された。
