Outline of Final Research Achievements

We have established a breath-hold radiotherapy technique by a visual feedback method using a wearable head-mount display and with the real-time positioning management system for image-guided radiotherapy. For patients of lung cancer or primary gastric malignant lymphoma (ML), we found that breath-holding (BH) at end-expiration showed a good reproducibility and good daily target coverage in BH-IGRT for gastric ML using in-room CT. Then we performed a planning study to avoid dosimetric risk factors for lung cancer: conventional radiotherapy versus intensity-modulated radiotherapy. A deep-inhalation BH technique using our method has been also introduced for left-sided breast cancer to minimize radiation-induced cardiac side effects.

Academic Significance and Societal Importance of the Research Achievements

高精度放射線治療は照射体積内の線量分布を飛躍的に改善し腫瘍の局所制御率の向上と正常組織の有害事象の低減を同時に実現可能であるが、I患者の体動とくに呼吸運動により大きな影響を受けて不確実な線量投与となり得る危険性があり、胸腹部悪性腫瘍に対する臨床応用を進める上での大きな障壁である。本研究では、汎用性の高い実用的な呼吸停止下照射システムの開発を達成した。胃原発悪性リンパ腫に対する呼気停止下画像誘導放射線治療、肺癌に対する強度変調放射線治療、左乳癌に対する心臓線量低減のための深吸気息止め照射など、順次臨床応用を開始し、着実な成果を上げている。
