Outline of Annual Research Achievements

頚動脈狭窄症(Internal Carotid Artery Stenosis:ICS)に起因する脳循環動態の悪化は認知機能障害の原因になると考えられている.ICSに対する手術(CEA/CAS)が認知機能を改善し得るか否か,安静時(resting state) functional MRI(rs-fMRI))を用いた神経ネットワークによる解析を利用して評価した.
対象は22例,各評価は,手術前,術後1週,術後3ヶ月に行った.認知機能評価にはMini Mental State examination(MMSE),Frontal Assessment Battery(FAB),日本語版Montreal Cognitive Assessment(MOCA)を使用したが,術後1週および3ヶ月のMOCAの改善を認めた.rs-fMRIの解析にはCONNを使用し,Independent Component Analysis(ICA)およびSeed-Based Analysisを行った.ICAでは,術後3ヶ月のprecuneus,anterior cingulate cortex,Seed-Based Analysisでは,術後1週のlt. lateral parietal region(LP),rt. inferior temporal gyrus(ITG),術後3ヶ月のbil. LP,bil. superior temporal gyrus(STG),bil. middle temporal gyrus,rt. ITGにおいて,神経ネットワークの有意な増加を認めた.
認知機能の改善と神経ネットワーク変化との相関を分析したところ,術後3ヶ月のMOCAの改善と,ICA解析のprecuneusおよびSeed-Based Analysisのrt. anterior STG-lt. posterior supramaeginal gyrus結合との間に有意な相関がみられた.
CEA/CAS治療により認知機能が改善することが示された.特に,術後3ヶ月の時点では,認知機能に関与する神経ネットワークの増加を伴うものであり,今回の結果によって,CEA/CAS後の認知機能改善の機序が示された.
