Outline of Final Research Achievements |
It is difficult to regenerate bone vertically that the alveolar ridge loss is due to be sever periodontitis or trauma. The cylindical plastic caps were used to observe vertical bone regeneration on rat calvarial bone. The top of cylindical cap was removed to change barrier permeability. Micro-CT volumetric analysis and decalcified hematoxylin and eosin-stained sections showed that GBA barrier permeability was inversely associated with the quantity of augmented bone obtained. Masson’s trichrome staining showed that collagen in newly generated bony tissue was more mature in plastic caps with three holes than in those with more-permeable or more-occlusive barriers. Bone augmentation was inhibited in specimens exhibiting invasion of soft tissue through penetrating holes, and barrier permeability was associated with the quantity of augmented bone developed. In conclusion, moderate barrier permeability is optimal for development of mature augmented bone.

Academic Significance and Societal Importance of the Research Achievements |
歯周病や外傷などによって,失われた歯槽骨を垂直方向に再生させることは臨床的に非常に困難である。これまでの研究からその原因として,垂直方向では骨再生に必要な血液や幹細胞などの因子が供給されないためであると考えた。そして本研究の結果,キャップ天井部からの骨膜利用が効果的なことが示された。また,遮蔽膜に適切な遮断性と透過性の両方が必要なこと,骨補填材を填入するとさらに骨再生が促進されることが解明された。本研究によって再生が困難な垂直方向の骨再生が常に十分に可能となれば,口腔内だけでなく体の外傷や骨折ならびに奇形,さらには臓器移植患者に対する再生・再建治療への応用も可能性が期待される。
