Outline of Final Research Achievements

The aim of this study was to apply telemonitoring of home oxygen therapy (HOT) to patients with chronic respiratory failure and to apply it to home-visit nursing. The following results were obtained: (1) We evaluated the changes in the scales of the health-related quality of life questionnaire (SF-36) before and 3 months after the telemonitoring intervention. Significant improvement was observed in the scales by telemonitoring of HOT. (2) A manual was made and published to improve the teaching efficiency for patients during home-visit nursing. (3) For patients of type II chronic respiratory failure with hypercapnia, we determined the inhaled oxygen flow rate that seems to be safe using a device to measure the percutaneous partial pressure of carbon dioxide at an outpatient clinic. It was suggested that a safer HOT could be constructed by determining the optimal inhaled oxygen flow in addition to telemonitoring of HOT with home-visit nursing.

Academic Significance and Societal Importance of the Research Achievements

在宅酸素療法は慢性呼吸不全患者さんの予後を改善する重要な治療法である.一方,患者さんが自宅などで短時間しか使用していない場合や,医師が処方した吸入酸素流量が適切でない可能性などに対する客観的確認は難しかった.本研究では遠隔モニタリング技術を利用することで,これらの臨床的問題点が解決可能であることを明らかにした.情報通信技術を用いて遠隔モニタリング可能にした在宅酸素療法の有用性を患者側からと訪問看護側から検討し,実践的な操作・指導マニュアルを作成した従来にはない研究であり,得られた情報を訪問看護師と共有して,よりよい医療が行える可能性を確認した.
