Morphological and Mechanical Properties of Achilles Tendon Twisted Structure-For Understanding the Mechanism of Achilles Tendon Disorder-

Outline of Annual Research Achievements

アキレス腱(AT)障害の発生メカニズムとしては,近年では,踵骨の回内時にAT内の歪みが不均一であることが要因として重要視されてきている.この原因としては,ATの特徴的な構造が関与している可能性が示唆されているが,ATの捻れの程度の違いを考慮して検討した報告はない.そこで本研究の目的は,踵骨を回内・回外方向に動かした際にATを構成する各腱線維束に加わる伸張度(%)を捻れのType毎に検討することとした.対象は,ATの3つの捻れのType(軽度,中等度,重度の捻れ)を1側ずつ(合計3側,平均年齢:83±18歳)を使用した.3DデジタイザMicroScribe装置を使用して,ATを構成する腓腹筋内側頭(MG)の停止する腱線維束,外側頭(LG)の停止する腱線維束,ヒラメ筋(Sol)の停止する腱線維束の筋腱移行部の最遠位端と踵骨隆起付着部の2点と,踵骨隆起(4点)をデジタイズして3D構築した.その後,任意に規定した踵骨隆起の回転中心を基準に作成した絶対座標系上で回内(20°)・回外(20°)方向に踵骨を動かした際の各腱の伸張度(%)をシミュレーションして算出した.解析には,SCILAB-5.5.0を使用した.どのタイプにおいても踵骨を回内すると,MG・LGは短縮,Solは伸張し,回外時にはMG・LGは伸張し,Solは短縮した.特に,TypeⅢでは,踵骨の回内時にSolの伸張度が最も大きく(TypeⅠ: Sol:1.7 ± 3.4%, TypeⅡ: Sol:2.4 ± 1.4%, TypeⅢ: Sol:3.7 ± 6.0%),更にSolを構成する各腱線維の伸張度が異なった.本研究結果より,踵骨回内時にはAT内に加わる伸張度は一様ではなく,特に,TypeⅢではAT障害の発生リスクが高まる可能性が示唆された.
