Outline of Annual Research Achievements

当研究は、中枢神経浸潤白血病を標的とした新規治療法の開発を目的として研究を進めた。まず、患者白血病細胞を移植したxenograftマウスモデルを用いて解析を行ない、CNS白血病細胞は骨髄白血病細胞と比較してVEGFAの発現が上昇していることを明らかにし、Bevacizumabを用いたCNS白血病に対する治療戦略を示した(Kato I, Nishinaka Y, Nakamura M et al. Hypoxic adaptation of leukemic cells infiltrating the CNS affords a therapeutic strategy targeting VEGFA. Blood. 2017;129: 3126-3129.)。
さらにはがん免疫療法として注目されるCART細胞療法を用いた中枢神経浸潤白血病に対する新規治療戦略を提示した。免疫不全マウスにALL細胞株SUSRを脳室内移植後、CART細胞を静脈内移植して治療効果を観察すると、5匹中2匹では腫瘍細胞の消失を確認したが、3匹のマウスで腫瘍残存、もしくは腫瘍再発を認めた。一方で、CART細胞を脳室内投与したところ大きな合併症無く6匹中、6匹で腫瘍消失を認めた。これらの結果からCART細胞を用いた中枢神経浸潤白血病に対する新規治療戦略を提示することが出来た。(Direct Delivery of piggyBac CD19 CAR T Cells Has Potent Anti-tumor Activity against ALL Cells in CNS in a Xenograft Mouse Model, Tanaka, K, Kato, I, Tanaka, M. et al. 臨床でその治療戦略に難渋することの多いCNS白血病に対して、PDXの長所を生かして多角的解析を行い、明らかにしたCNS白血病の特徴を標的とした新規治療戦略を提示すことが出来た。
