Outline of Final Research Achievements

This study was enrolled 7 patients diagnosed at Japanese spotted fever (JSF) under the obtained enough informed consent. The 7 patients was proved at JSF by peformed the PCR test from blood and/or skin or acute stage IgM>80 against anti-R.japonica antibody or IgM and/or IgG more than 4 times between acute and recovery stage.
We messured any cytokines: IFN-γ, TNF-α, TNF-β, IL-1β, IL-2, IL-4, IL-5, IL-6, IL-8, IL-9, IL-10, IL-12p70, IL-17A, IL-17F, IL-22, IL-33, GM-CSF, TSLP at the first visit (acute stage) and 14days after treatment (recovery stage).
Case 1,2,3 were measured at 4 points: 1, 3, 7, 14 day. The cytokines were tested by flow cytometric analyses using AimPlex. And We also compeared the asossiation of WBC, Eosin and CRP.

Academic Significance and Societal Importance of the Research Achievements

リケッチアは血管内皮細胞に移動し寄生感染するためIFN-γを誘導する。
ゆえに急激なIL-6の刺激でほとんどのナイーブT細胞がまずTh1系に分化し、IL-12を介し細胞内寄生菌に対し攻撃するIF-γに分化しようとするため、Th-17系への分化の優先度は低く好中球は微増となると推測される。またよりTh2系へのサイトカイン放出の優先度は低くなるため、好酸球分化はなくなり消失すると推測する。Th2系自然免疫に関与するIL-4、IL-5、IL-33などは急性期と回復期に優位差はないことが判明した。
R.japonica感染においてはIL-6、IFN-γが感染防御にきわめて重要なサイトカインとなっている。
