バイオ３Dプリンタによるヒト脂肪幹細胞小口径血管の研究

Outline of Annual Research Achievements

インビトロにおけるヒト脂肪幹細胞小口径血管の作成に関しては、直径2mm程度とより細く、長さ5cm程度の長い血管を作成するための細胞の組成の調節を行っている。血管の質感に関してはNHDF(繊維芽細胞)のみでの血管作成の工夫を行っている。また、血管作成時の成長促進因子の検討として細胞に力学的なストレスを与えるなどして、血管強度を上げる検討を行っている。
3Dプリンタに関しては細胞の3次元細胞構築と並行し、高強度PLA(ポリ乳酸)樹脂を使用した頭蓋骨モデルを作成している。現在、患者一人一人のオーダーメイドでの治療への需要が高まっており、新しい画像支援システムの開発を行っている。HICTOP 3DプリンターReprap Prusa i3 高精度 大容量造形サイズ 工業級3Dプリンター (3DP-22)を用いて、オーダーメイドの頭蓋・顎顔面骨モデルを作成し、術前にシミュレーションを行っている。また、CT・MRI画像データを術前に編集・加工・合成する事で手術のシミュレーションを行う研究を行っている。3Dプリンタによる種々の手術支援に関しては後頭頭蓋拡大術における静脈洞と頭蓋骨の三次元透見合成画像の有用性の研究を行っており、3Dプリンタの精度の研究を行い、2回の学会発表を行った。
また、臨床的には遊離組織移植に関係する海外発表として2020年の1月13日にアメリカマイクロサージャリー学会(アメリカ・フロリダ)ASRM 2020 Annual meetingにおいて 、Volumetric changes of transferred free anterolateral thigh flaps in head and neck lesions の演題を発表を行った。

Reason

共同研究員として、大学病院以外での勤務時間が長く、細胞培養での血管作成やラットへの移植実験が滞っている。但し、3Dプリンタによる種々の手術支援に関しては後頭頭蓋拡大術における静脈洞と頭蓋骨の三次元透見合成画像の有用性の研究を行っており、3Dプリンタの精度の研究を行い、2回の学会発表を行った。年度末はコロナ感染症に伴い、研究室への移動等が制限され、研究遅延を来している。

Strategy for Future Research Activity

人工血管の作成に関してはコロナ感染症にともに、実験施設での研究推進がさらに困難となると思われる。3Dプリンタを用いた後頭頭蓋拡大術における静脈洞と頭蓋骨の三次元透見合成画像の有用性の研究など新たな課題へのチャレンジを続けていきたい。今後はCADシステムによる3Dモデルによる骨切り手術の支援システムを作成し、さらなる精度の高い手術支援システムの作成を検討している。

Causes of Carryover

予定しいた血管作成が予想していた結果を残せていないため、計画を推進したい。また、現段階までの研究結果を学会発表・論文化する必要があるため。
